PHILIPPINE PASSPORT APPLICATION REQUIREMENTS – Here are the documents required in applying for a passport to the Department of Foreign Affairs (DFA).

More popularly called DFA, the Department of Foreign Affairs issues passport to both adults and minor applicants provided that they passed the qualifications and the documentary requirements. It may take a few weeks before you can get your passport since you submitted the Philippine passport application requirements.

The Philippine passport comes with a validity. Thus, it must be renewed from time to time to remain active and acceptable in traveling abroad. It is required even in visiting no visa countries.

The passport can also be used as a primary identification document as well like in applying for a loan, filing for a social insurance claim, making a huge purchase, etc.

The Philippine passport application requirements among adult and minor applicants vary. However, it is important to remember that the Department of Foreign Affairs requires online appointment and the applicant must visit the Consular Office at the appointed date and time.

For adults, here are the documents that the DFA requires in applying for a Philippine passport:

  • Personal Appearance
  • duly-accomplished Passport Application Form
  • original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
    • Married Females (who are using their spouse’s last name) must also present Original and submit photocopy of PSA Authenticated Marriage Contract on Security Paper or Report of Marriage
    • Local Civil Registrar Copy is required if PSA Birth Certificate is not clear or cannot be read
  • any of the following acceptable IDs with one (1) photocopy
    • Philippine Identification (PhilID)/ePhilID
    • Social Security System (SSS) Card
    • Government Service Insurance System (GSIS) Card
    • Unified Multi-Purpose Identification (UMID) Card
    • Land Transportation Office (LTO) Driver’s License (Driver’s License cards expiring on 24 April until 31 October 2023 and said expired cards perforated during the renewal process are accepted together with the temporary license in paper format).
    • Professional Regulatory Commission (PRC) ID
    • Overseas Workers Welfare Administration (OWWA) E-Card
    • Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila.
    • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
    • Senior Citizen ID
    • Airman License (issued August 2016 onwards)
    • Philippine Postal ID (issued November 2016 onwards)
    • Seafarer’s Record Book (SRB) or Seafarers Identity Document (SID) (*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
    • Valid or Latest Passport (For Renewal of Passport)
    • School ID (if applicable)

For minors, the Philippine Passport application requirements includes documents of both the minor and his/her parents. Here are the documents that must be submitted to DFA in applying for a passport:

  • Confirmed Online Appointment
  • duly-accomplished Passport Application Form
  • Personal Appearance of minor applicant and either parent or authorized adult companion
  • Philippine Statistics Authority (PSA) Birth Certificate
    • Local Civil Registrar Copy may be required if PSA Birth Certificate is not clear or cannot be read
  • Marriage Certificate is required if only one parent is accompanying the child
    • If parents are unmarried, a Special Power of Attorney (SPA) executed by the Mother will be required if she is not accompanying the child
    • If Special Power of Attorney (SPA) is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
  • school ID
  • Passport or Valid Government issued ID of either parent with one (1) photocopy
