List of PWD ID Requirements You Must Prepare & Submit to Apply
PWD ID REQUIREMENTS – Here are the documents required in applying for an ID for persons with disability (PWDs).
Among the situations that hamper an individual from living a normal life is a disability. It can be a recurring mental problem, the inability to walk or see, the loss of two hands following an accident, and a lot more. Whether it is in-born or a result of an accident, its impact can range from causing the person to live a different kind of life to an inability to function normally. Nevertheless, these disabilities never affects the value a person has.
In the Philippines, the government releases a PWD ID that individuals who have disabilities may apply for. Upon the presentation of a PWD ID, the cardholder may qualify to a lot of benefits and privileges including exemptions from the Value Added Tax (VAT) and other discounts. This is apart from a priority lane for PWDs in many establishments.
To apply for this type of ID set for the individuals with disabilities, there are PWD ID requirements for application. Here is a list of the documents that you must prepare to apply for the ID:
- duly-accomplished PWD ID Application Form which is available from any of the following:
- Department of Social Welfare and Development (DSWD) offices
- National Council on Disability Affairs (NCDA) or its regional counterpart
- Office of the Mayor
- DOH Online Registration System
- PWD ID application form from the DOH website
- Participating organizations with memorandums of agreement with the DOH
- Office of the Barangay Captain
- any valid government-issued ID
- Two (2) recent 1×1 ID photos with name and signature or thumb marks at the back
- latest school assessment/medical certificate/or disability certificate confirming disability condition from government facility or licensed private clinic or medical professional
You may submit your PWD ID requirements for application at the nearest NCDA office or any government office in your city/municipality that process the issuance of this type of personal document. For first time applicants, it will be free of charge.
The PWD ID is valid for five (5) years. The renewal comes at a low cost.
