Guide on the GSIS Survivorship Claim Requirements for Deceased Member’s Family
GSIS SURVIVORSHIP CLAIM REQUIREMENTS – Here are the documents needed in filing a Survivorship Benefit claim to the Government Service Insurance System (GSIS).
Undeniably, among the many benefit offers of the Government Service Insurance System (GSIS) for its members, the one benefit that most members look forward to the most is the Retirement Benefit. It is where the monthly pension is given to a retired member. However, truth be told that not everyone gets to the point.
Meanwhile, there is another relief in case a GSIS member won’t get to the age of retiring from work – the GSIS Survivorship Benefit. The said offer targets to benefit the family, specifically the legal spouse and the dependent minor children, of the deceased GSIS member.
Here is the order of beneficiaries observed by the GSIS in the granting of the claim:
- Primary Beneficiaries:
- Legal spouse until he/she marries again, co-habits or engages in a common-law relationship
- Legal, legally adopted or legitimated children, and illegitimate children of the deceased GSIS member who are minors or have reached the age of majority but still incapable of self-support
- Secondary Beneficiaries:
- Parents of the GSIS member
If the GSIS member who died was not married and has no children, the GSIS will give the Survivorship Claim to the parents who are his/her beneficiaries. In case the deceased member has no legal spouse but has children, the decision of the children will take an upperhand on the granting of the survivorship benefit to his/her parents.
The requirements in claiming the GSIS Survivorship benefit are different from the GSIS Funeral Benefit requirements, GSIS Unemployment Benefit requirements, and GSIS Disability Benefit requirements.
The civil status of the deceased member of the Government Service Insurance System is a major factor in determining the GSIS Survivorship Claim requirements that must be submitted by his/her beneficiaries:
For the beneficiaries of a single deceased GSIS member, prepare the following:
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- If with minor/incapacitated children, Affidavit of Guardianship
- If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD; and
- If guardian is not a GSIS member, Birth Certificate issued by NSO
For the beneficiaries of a married deceased GSIS member, prepare the following:
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- Marriage certificate between the deceased and his/her spouse issued by NSO
- Affidavit of Surviving spouse
- If spouse is not a GSIS member, Birth Certificate issued by NSO
- Birth Certificate/s of minor/incapacitated children issued by NSO
- If with minor/incapacitated children, Affidavit of Guardianship
- If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD
For the beneficiaries of a GSIS member who has no primary beneficiaries but either or both of his/her parents are alive, prepare the following:
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- Birth Certificate of member issued by NSO
- Marriage Contract of member’s parents issued by NSO
- Birth Certificate of member’s parents issued by NSO if parents are not GSIS members/pensioners
For the beneficiaries of a GSIS member who has no primary and secondary beneficiaries but at least one of his siblings is alive, prepare the following:
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- Birth Certificate of member issued by NSO
- Marriage Contract of member’s parents issued by NSO
- Death Certificates of member’s parents issued by NSO
- Birth Certificate of member’s parents issued by NSO
- Affidavit of Surviving Legal Heirs stating among others that affiants are the only surviving legal heirs of the deceased member, their date of birth and relationship to the deceased member and that they are executing the document for the purpose of claiming the benefit from GSIS
- Birth Certificate of all the siblings of the deceased member issued by NSO
- Marriage Contract of all the female siblings of the deceased member issued by NSO
- If with deceased brother or sister, extra judicial settlement among the legal heirs of the deceased brother / sister of the member designating one payee
