With regards to the GSIS Retirement Benefit requirements, it depends on which Retirement Law the retiring member of the government agency will be under in.

For retirees under the Republic Act No. 660, prepare the following documents:

  • duly-accomplished application form for Retirement/Separation/Life Insurance Benefit
  • Service Record with Leave without Pay (LWOP) certification (indicating specific time and dates of LWOP)
  • Declaration of Pendency/ Non-Pendency of Case (DPNPC) form (date administered/ notarized should be on or after receipt of notification from GSIS.

For retirees under the Republic Act No. 1616, prepare the following documents:

  • duly-accomplished application form for Retirement/Separation/Life Insurance Benefit
  • Service Record with Leave without Pay (LWOP) certification (indicating specific time and dates of LWOP)
  • Declaration of Pendency/ Non-Pendency of Case (DPNPC) form (date administered/ notarized should be on or after receipt of notification from GSIS.

For retirees under the Presidential Decreen No. 1146, prepare the following documents:

  • duly-accomplished application form for Retirement/Separation/Life Insurance Benefit
  • Service Record with Leave without Pay (LWOP) certification (indicating specific time and dates of LWOP)
  • Declaration of Pendency/ Non-Pendency of Case (DPNPC) form (date administered/ notarized should be on or after receipt of notification from GSIS.

For retirees under the Republic Act No. 8291, prepare the following documents:

  • Certification of SSS premium contributions indicating number and inclusive months of contributions signed by authorized SSS Officer

  • Declaration of Pendency/ Non-Pendency of Case (DPNPC) form (date administered/ notarized should be on or after receipt of notification from GSIS
