How To Apply To Be SSS Member as Employee & the Requirements
APPLY SSS MEMBER AS EMPLOYEE – Here is a guide on the process in applying for a membership to the Social Security System.
One of the giants in terms of social insurance in the Philippines is the Social Security System or more popularly called SSS. Millions of Filipinos are members of this state entity that has crafted several offers that are exclusive for their members.
To apply to be an SSS member is one of the mandates among the employees in the private sector. An SSS membership is required by most private companies and businesses in the country in the pursuit of widening the resources of their workers and helping them prepare for the future.
In the case of the locally-employed individuals who are members of the Social Security System, the employer pay for a portion of the SSS contribution. There are only a few steps on how to apply to be an SSS member if you are an employee of a private company.
Step 1 — Prepare the documents required by the Social Security System for the SSS membership application:
- duly-accomplished SSS Form E-1 (Personal Record)
- an original (or Certified True Copy) and a photocopy of any primary document such as any of the following:
- Passport
- Birth or Baptismal Certificate
- Driver’s License
- Seaman’s Book
- Professional Regulation Commission (PRC) Card
- you may also submit two (2) of any of the following secondary documents:
- ATM card with the cardholder’s name
- Bank Account Passbook
- Birth or Baptismal Certificate
- ATM card and certification from the issuing bank if the account number doesn’t bear the cardholder’s name
- Alien Certificate of Registration
- Certificate from various authorities
- Certificate of Licensure, Qualification Document, or Seafarer’s ID from the Maritime Industry Authority
- Certificate of Naturalization from the Bureau of Immigration
- Company ID
- Company Representative Authorization Card issued by SSS
- Credit Card
- Fisherman’s Card issued by the Bureau of Fisheries and Aquatic Resources
- GSIS Card or Certificate of Membership
- Health or Medical Card
- ID card issued by professional organizations recognized by the PRC
- Life Insurance Policy
- ID card issued by Local Government Units
- Marriage Contract
- NBI Clearance
- Pag-IBIG Member’s Data Form
- Membership card issued by private companies
- Overseas Workers Welfare Administration card
- PhilHealth Member’s Data Record
- School ID
- Police Clearance
- Postal ID
- Senior Citizen Card
- Seafarer’s Registration Certificate issued by the Philippine Overseas Employment Administration
- Tax Identification Number (TIN) Card
- Transcript of School Records
- Temporary License and Student Permit issued by the Land Transportation Office
- Voter’s Identification Card or Affidavit
Step 2 — Visit a branch of the SSS and submit your duly filled-out SSS Membership Application Form and the other requirements.
Step 3 — Pay for the initial contribution payment to the SSS. The amount is based on your monthly salary.
Step 4 — After the processing, the SSS will give you your SS number. Keep your records safe.
