REQUIREMENTS FOR UMID ID APPLICATION – Listed below are the documents required in applying for the Unified Multi-purpose ID card.
Have you heard about the Unified Multi-Purpose ID card or more popularly called UMID ID? It is one of the most beneficial ID cards that a person can have as it can be used in processing transactions in four (4) of the biggest government agencies in the country.
The Unified Multi-purpose ID card is issued by the Social Security System to its members who have posted at least one (1) monthly contribution to their accounts. It replaced the SS ID. It is a unified valid document that can be used for processing transactions in the following government offices:
- Government Service Insurance System (GSIS)
- Social Security System (SSS)
- Pagtutulungan sa Kinabukasan: Ikaw, Bangko, Industriya, at Gobyerno (Pag-IBIG) Fund
- Philippine Health Insurance Corporation (PhilHealth)
If you have a UMID ID, you may use it not only in applying for bank loans and processing a passport or buying a car or a residential property but as well as in processing transactions at other government offices including the Bureau of Internal Revenue (BIR).

- duly-accomplished and signed UMID ID Card Application Form
- original and photocopy of any of the following primary valid IDs:
- SSS card
- Passport (See Philippine passport application requirements)
- Voter’s ID
- LTO Driver’s License
- Postal ID
- NBI Clearance
- Seaman’s Book
- In the absence of any primary valid ID, you may submit an original and photocopy of two (2) secondary IDs. Both of the IDs presented must bear your signature and at least one (1) of them must have your photo. Here is a list of the IDs you may present:
