REQUIREMENTS FOR DSWD 4PS APPLICATION – Here are the documents needed in applying for the DSWD Pantawid Pamilyang Pilipino Program (4Ps).

The Department of Social Welfare and Development (DSWD) is the government agency in the Philippines which runs several programs to aid Filipinos in their financial needs. One of these is the Pantawid Pamilyang Pilipino Program (4Ps) which is one of the biggest anti-poverty programs in the country.

There are factors that the DSWD consider in approving applications of aspiring beneficiaries of 4Ps. The program was really crafted to address the needs of the Filipinos who belong to the poorest of the poor thus the following factors are considered:

• a resident of one of the poorest communities in the Philippines
• a part of the household whose economic status is below the provincial poverty level
• having children between 0 and 18 years of age or a pregnant member of the family when the assessment was made

There are documents that you need to gather in applying to be a beneficiary of the DSWD 4Ps. The following are the requirements for DSWD 4Ps application:

• copies of birth certificates of all family members
• copies of school IDs or first grading period school report cards of all children in the family who attended school
• barangay certificate
• valid IDs of the family member attending the community assembly
• 2 1×1 picture with white background
• copy of health records of 0-5 years old member of the family from the health center where they have their check-ups

For the selection of the beneficiaries, the list of poor households are submitted to the PMED of the Department of Social Welfare and Development (DSWD) 4Ps NPMO for evaluation and planning. There will be assessments of the individual eligibilities to the program.
