DSWD REQUIREMENTS FOR CASH AID – Here are the documents needed in applying for cash assistance to the Department of Social Welfare and Development (DSWD).
The Department of Social Welfare and Development, more commonly called DSWD, is an entity that Filipinos may turn to in time of financial needs. It has crafted several programs for the poorest of the poor.
One of the programs of the DSWD is the Assistance to Individuals in Crisis Situations or also called as AICS. It was crafted to help Pinoys in crisis situations like medical emergencies or was struck by a calamity. Under it, there are medical cash assistance, burial assistance, transportation aid, and educational assistance. There are different DSWD requirements for cash aid depends on the type of assistance you need.
DSWD Requirements for Cash Aid
For medical or hospital assistance, prepare the following documents:
- Clinical Abstract and/or
- Social Case Study Report from LGU
- Referral/Endorsement letter from legislator, if applicable
- Barangay Certification/Indigency/ any valid ID of the patient or his/her representative
- Laboratory Request/Medical Procedures (recommended by the attending Physician)
- Prescription (for medicine)
- Medical Certificate with license/PTR No. of the attending physician
- Billing Statement (for hospital bill)
For educational assistance, prepare the following documents:
- Barangay Certification/Indigency/any valid ID
- Social Case Study Report (optional)
- Certificate of Enrollment
- Referral/Endorsement letter from legislator, if applicable
For burial assistance, prepare the following documents:
- Funeral Contract
- Registered Death Certificate
- Barangay Certificate/any valid ID
- Referral/Endorsement Letter from legislator
- Permit to Transfer
- Social Case Study report from LGU (optional)
For transportation assistance, prepare the following documents:
- Referral/Endorsement letter from legislator, if applicable
- Barangay Certification/Indigency/any valid ID
- Social Case Study Report from LGU (optional)
In applying for a DSWD assistance, go to a branch of DSWD and submit your documents for evaluation. All applications are subject to the approval of the government agency.
