REQUIREMENTS FOR DSWD MEDICAL ASSISTANCE 2023 – Listed below are the documents needed in applying for a Medical Aid to the Department of Social Welfare and Development.
In the Philippines, one of the government agencies that have several programs that benefit the Filipino people most especially the poor and the underprivileged individuals is the Department of Social Welfare and Development. It is more popularly called DSWD.
The government agency has several offers. Aside from the DSWD cash aid offers, the government agency also offers a Medical Assistance for the poor Filipinos and the indigents who are caught in the midst of a medical situation.
To apply for the aid, there are documentary requirements for DSWD Medical Assistance that you must prepare and submit to the government agency. Here are some of the documents needed:

For Hospital Bill Assistance
- Temporary / Final Hospital Bill/Statement of account with the complete name and signature of the billing clerk.
- Certificate of Balance or Promissory Note if the patient has been discharged from the hospital and there is still an unpaid balance.

DSWD noted that the date when the requirements for the DSWD Medical Assistance were issued must be indicated as well as the complete name, license number, and signature of the attending physician. Furthermore, the documents must have been issued within three (3) months.
In case the medical aid goes above Php 10,000.00, the assistance to be extended will be in the form of a Guarantee Letter and there is a need for a Social Case Study Report (SCSR) from the City Social Welfare and Development Office or CSWDO.
