It is believed that this idiom originated in an old English tradition in which a third bell, called the Lych Bell, is usually rung at a burial. A passing bell was rung in that custom to notify a clergyman that a person was dying and to enable them to be brought to their bedside.

The Death Knell was formally written in law under the reigns of Henry VIII and Elizabeth I. The knell (bell) was sounded differently in each parish, sometimes three times—three bells for men, and three times—two bells for women—to denote the deceased’s age and other times to denote their gender.
