In a statement, PhilHealth President and CEO Dante A. Gierran on Tuesday said that they urged their members to know and keep their PIN in order to prevent delays and problems during vaccination.

Gierran also said that those who didn’t know their PhilHealth ID number or not yet registered with PhilHealth should coordinate with any PhilHealth office or should call them so that they can provide them their PhilHealth Identification Number or facilitate their registration.

In order to avoid delays and problems during vaccination, we encourage our members to know and keep their PIN. For those who do not know their PIN or not yet registered with PhilHealth, they should coordinate with any PhilHealth office or call us so we can provide them their PIN or facilitate their registration,” Gierran said.

Also in a statement, PhilHealth said that DOH Memorandum 2021-0099 stated that all potential vaccine recipients needed to be registered using unique identifiers as identified during the masterlisting process such as but not limited to “full name and birthday, PhilHealth identification number, system-generated alphanumeric or QR or unique codes, or similar.”

In addition, potential vaccine recipients could also present any government-issued identification card or government document.

Non-PhilHealth members have to fill out the PMRF or the PhilHealth Member Registration Form that can be downloaded here. The accomplished PMRF and a clear photo or a scanned copy of any valid identification card can be sent to [email protected], with the subject: Register Name City/Province, Region.

After processing, the PhilHealth Identification Number will be sent to the e-mail address in the registration form.
